Statement of Purpose

Emergency Services Case Management provides comprehensive case management services to the homeless population, low income families and single adults. It serves to foster client self-actualization and provide them with the tools and resources to achieve self-sufficiency.


Services

NEIGHBOR TO NEIGHBOR PROGRAM (UTILITY ASSISTANCE)

  • Final disconnection notice needed in order to apply.

EMERGENCY SENIOR LOAN PROGRAM

  • Qualification include : 60 years of age, Proof of Emergency (supportive documentation), Proof of Income.

WINTER ROTATIONAL SHELTER

  • All clients must take a phone pre-screening assessment in order to qualify for program. Qualification include : Valid ID and Weekly Meetings.

HOTEL VOUCHER PROGRAM (SEASONAL)

  • Qualifications include : Homeless, Source of Transportation, Valid ID and Weekly Meetings.

RENTAL ASSISTANCE PROGRAM (SEASONAL)

  • Qualifications include : Valid ID, Rental Agreement, Last 2 Rent Receipts, Proof of Income and 3 day notice or letter from mortgage company.

LIONS VISION PROGRAM AT REDUCED RATE

  • Lions Vision is a reduced fee voucher referral program.  It is designed to assist the visually impaired, blind or faced a similar medical condition. Qualifications include : Proof of Income.

DMV CA ID VOUCHER AT REDUCED FEE

  • Reduced fee voucher for low income families. Qualifications include : Income Verification and Address Verification.

SECURITY DEPOSIT PROGRAM/CO-SIGNERS

  • Qualifications include : Zip Code beginning in 921_ _, Proof of Income, 3rd party check MUST be accepted by landlord, Inspection letter.
